A Case of Rhabdomyolysis due to Hyponatremia in a Patient with Schizophrenia

Abstract

Hyponatremia is rarely reported to cause rhabdomyolysis. We experienced a case of rhabdomyolysis due to polydipsic hyponatremia in a patient with schizophrenia. A 37-year-old man with a 6-year history of schizophrenia had been treated with risperidone. He was admitted to our hospital with a reduced conscious level. He had polydipsia and his initial blood chemistry revealed severe hypotonic hyponatremia. After correction of hyponatremia with intravenous 3% saline, he developed a substantial elevation in the creatine phosphokinase level without any evidence of muscle trauma, seizure, or any signs of neuroleptic malignant syndrome. He recovered after treatment by isotonic saline hydration, urine alkalization and conservative management.
